ACM Care is Committed to Listening to Your Concerns
Listening is the first step in developing a relationship.
We will arrange a complimentary phone consultation to discuss whether our services are a good fit for you or if a simpler, more cost-effective solution is available. If you don’t reside in our service area, we will give you referrals and contacts to providers outside the Chicago, Peoria, and St Louis regions.
Once you decide ACM Care’s mission is a match for your needs, we will provide you with a quote for a price based upon the ACM Care services you select.
Then we will schedule an initial onsite appointment with the prospective client and any family members who want to to be present.
What To Expect From This Point Forward
1.Attending the initial appointment during which your Care Manager will review the selected ACM Care services with you. We will obtain your signature on the service agreement of ACM Care, and the required HIPAA release forms. We will answer any initial questions you may have and discuss current challenges your loved one is facing from each parties’ point-of-view. This discussion allows us to take stock of all of the personalities involved to better customize our services. We also use this time to determine the goals for our involvement in your loved one’s care.
2.After this assessment, or soon afterwards, we will customize an ACM Care action plan for you & your loved one. The plan will outline the exact services we will provide, as well as how often these services will be delivered. We may also provide (unbiased) information on community resources for services that we do not provide but feel your loved one may find beneficial.
3.Once the ACM Care action plan is agreed upon by all parties, we will begin working toward our goals. We take great pride in building long-term, trusting relationships with our clients and their families. We will communicate our progress and update you on the status of our work on a regular and as needed basis.
