1.We meet with the client and any family members who wish to be present. At this initial appointment we will answer any questions you have, review HIPAA and confidentiality standards, and determine what the goals for our involvement with your family will be.
2.Based on our initial assessment and meeting, we will develop an individualized ACM Care action plan. It will outline the services we will provide, and how often these services will be delivered.
3.Once the collaborative plan is established, we begin working on the goals we have set.
ACM Care is a non-profit organization dedicated to providing quality senior care and disability care support services in St Louis, Chicago, and Peoria. We help protect the dignity, confidentiality and safety of our clients, in whatever setting they call home.
Our experienced staff is familiar with the best care options for disabled adults and the elderly in Illinois and Missouri. We use that knowledge to develop a custom care solution that best meets your loved one’s individual needs. Then we provide on-going support to help each client reach and maintain their highest quality of life. The nurses and geriatric case managers on our team can oversee every detail you want help with from medication management to making sure bills are paid.
The ACM Care process begins with listening. In our complimentary phone assessment, we will discuss your loved one’s unique situation and the concerns you have for them. If you decide we would be a good fit for your family, we begin the process of finding the best elder care solution.
